Community Mental Health Teams include community psychiatric nurses, occupational therapists and assistants, psychologists, approved social workers, support workers, psychiatrists and staff grade doctors. The teams work with patients in their own homes, clinics and the Community Mental Health Resource Centres. The teams carry out comprehensive health and social care assessments and prepare, implement and evaluate care plans in accordance with the Care Programme Approach. All initial assessments are carried out by a qualified worker e.g. community mental health nurse, social worker, occupational therapist or other professional as appropriate.
